ICAP’s donation will fund the continuation of the successful Family Action Plan project which supports families of patients who have been discharged from a period of hospitalisation. Educating low-income families about healthy living, work ethic and good citizenship helps break the poverty cycle and reduces the chances of further hospitalisation.

Charity Overview

Associação Saúde Criança, founded in 1991 is a non-profit organisation which supports children and families from low-income backgrounds following a hospital discharge. Patients are referred to the organisation by a team of multidisciplinary medical professionals to ensure aftercare is available once patients return to their home environment to limit the risk of a health relapse and readmission to hospital. Saúde Criança works with families to create a customised Family Action Plan which focuses on five different areas of well-being: health, income, housing, education and citizenship.
